DPC Primary Care Members, the INITIAL process is scheduling a ONE TIME Medical Consultation for medical needs such as Hormone and/or Wellness Consults,
Weight Loss,Chronic Medical Consultations (second opinions), etc . Membership options are reviewed at the time of initial visit.
OB/Prenatal Care/Delivery Fees:
$169 per Month x 9 Payments:
Includes Pre/Post-Natal Care, as well as Vaginal Delivery.
$500 Surgical Fee Added for Cesarean Deliveries.
*OF NOTE: Membership to the DPC is NOT required to become an OB Patient of Dr. B at the HH.

**At the Healthy Human, we do our very BEST to keep you out of the Emergency Room at all costs.
Next Day or Same Day Virtual/Clinic Visits with HH Team
Discounted Services (NOT INCLUSIVE in Membership Fees) for Members:
In-House Pharmacy (Generic meds from $2-10) to save on your medications
Extremely Reduced Labs & In-House Lab Draws (80% LESS than traditional lab fees with or w/o insurance)
Discounted Procedures: $80-120 for each joint injection, mole removal, etc.
In-Clinic After Hours (After 5pm M-F; Sat/Sun) Visits for Members: $100/Visit
Home Visit within 10 miles of Clinic for Members: $199/Visit
Billing & Payment
ONE TIME ENROLLMENT FEE: $100 Individual, $150 Family
A one-time, non-refundable enrollment fee will be charged to each new billing account.
MEMBERSHIP FEES
Our membership fee(s) are charged at the beginning of the month (1st & 5th) for services provided and are billed on an ongoing basis. There will be a "failed" payment fee of $25 added to your monthly bill, if your charges are not processed on the date per your contract. Payment of membership fees is required to remain an active member and receive service.

ALL members must enroll for Auto-Pay of invoices through credit card/debit card/checking account.
Memberships require NO long term contracts.
If you cancel and wish to re-enroll, it will require a $200 Re-enrollment fee.
SERVICE FEES
All services not covered by membership fees (e.g. after-hours visit, labs, meds, procedures, etc.) are required
to be PAID IN-FULL AT THE TIME OF SERVICE unless you have made arrangements with our staff.
Members may pay by check/cash or credit/debit/HSA card.
AFTER HOURS/WEEKEND FEES
If HH Physician/Provider is available on the weekend for an exam/assessment of
acute/urgent needs, to rid you from a costly Urgent Care or ER visit,
please be aware that you will be billed $110 for the After Hours Visit PLUS your monthly membership.
NO-SHOW FEES
All cancellations for scheduled visits must be done a minimum of 4 hours prior to scheduled time.
If you do not show for your appointment, or are excessively late for your scheduled time,
$25 fee will be charged for the missed visit.
Exceptions may be granted at our discretion.
CANCELLATION
You may cancel your membership at anytime, for any reason, by e-mail to contact@healthyhumandpc.com.
Your 30 day cancellation notice, does NOT begin until we have written termination through our signed Termination form. At the time of cancellation, by patient choice or recurring delinquent funds, you will have 30 days to find a new Primary Care clinic for your medical needs. During this period, you will continue payment for monthly memberships PLUS any remaining account balance. HH DPC will offer Urgent Care needs only during this 30 day period.
**Full details are included in The Healthy Human Patient Agreement and can be reviewed when the patient enrolls online. Patient is also responsible to sign the contract in person at the first appointment.

Our goal is to take care of you ALWAYS! This means you can start your care here before pregnancy, and we will provide your obstetric needs, including delivery.
Dr. Beckenhauer has privileges to deliver and care for you at MCH&HS (Blair). She can deliver you vaginally or by Cesarean Section at this location.
We provide personalized pediatric care that your child deserves. As healthcare providers, and even more importantly as parents, we want easy access for medical care when our loved ones are ill. At the Healthy Human, you are able to reach us easily to address your needs.
We get it, being healthy does't come overnight! We also understand that losing weight cannot happen unless our bodies are in BALANCE. We also carry Physician prescribed supplements and the Revision, and OBAGI, Skincare line to help you achieve your health and beauty goals as well!
If you currently have an illness and would need IV fluids, we are here to help you get back on your feet again as quick as possible. Dr. Beckenhauer has worked in an Emergency Room setting and she understands the fees and wait time. Just call/email/text us, we will have you come in and determine if it is appropriate for you to receive them at our clinic.
