$299 Initial Consultation, Follow Up Visits $249-299.

After the Initial Consult and Follow Up, Dr. B or Sandy will review membership options, IF they are available.
When memberships are available:
Adults (19+): $119/mo
Children (0-18) & College Students (19-24): $59/mo
*valid Student ID at enrollment

HH One-Time Enrollment Fee:​

OB/Prenatal Care/Delivery Fees:
OB + Primary Care Medical Services
$219 per Month x 9 Payments
Includes Pre/Post-Natal Care, Hospital Care, as well as the Vaginal Delivery Fee.
$500 Surgical Fee Added for Cesarean Deliveries.
OF NOTE:
Membership to the DPC is NOT required to be an OB Patient at the Healthy Human locations.

MEMBERSHIP FEES:​
Our membership fee(s) are charged at the beginning of the month (1st & 5th) for services provided and are billed on an ongoing basis.
ALL members MUST enroll for Auto-Pay of invoices through a valid credit card/debit card/checking account.
​
If you cancel and wish to re-enroll, it will require a $200 Re-enrollment fee. ​
​
*Memberships require NO long term contracts.
FAILED PAYMENT FEES:​
A failed payment fee of $25 is added to your monthly bill, if your charges are not processed on the date per your contract.
​
Payment of membership fees is required to remain an active member and receive service.

SERVICE FEES:​
All services not covered by membership fees and will be required to be PAID IN-FULL AT THE TIME OF SERVICE unless you have made pre-arrangements with our staff or Dr. B.
Members may pay by check/cash or credit/debit/HSA card. ​
AFTER HOURS or WEEKEND FEES:​
If an HH Physician/Provider is available on the weekend for an exam/assessment of acute/urgent need, to rid you from a costly Urgent Care or ER visit, please be aware that you will be billed $110 for the After Hours Visit PLUS your monthly membership.
NO-SHOW FEES:​
All cancellations for scheduled visits (Physician and/or Nursing) must be done a minimum of 4 hours prior to scheduled time.
If you do not show for your appointment, or are excessively late for your scheduled time a fee of $25 will be charged for the missed visit.
Exceptions may be granted at our discretion.
CANCELLATION:
You may cancel your membership at anytime, for any reason, by e-mail to contact@healthyhumandpc.com.
Your 30 day cancellation notice does NOT begin until we have written termination through our signed Termination form that is available within 48 hrs of request, if it is not over a holiday/vacation period. Please call the clinic if you email a request and do not her back from us within 48 hrs.
At the time of cancellation, by patient choice OR Physician termination due to recurring delinquent funds (HH Policy of 2 months+ of unpaid invoices will result in a cancellation of your services), you will have 30 days to find a new Primary Care clinic for your medical needs.
During this period, you will continue payment for monthly memberships PLUS any remaining account balance.
HH DPC will offer Urgent Care needs only during this 30 day period.
*Full details are included in The Healthy Human Patient Agreement and can be reviewed when the patient enrolls online. Patient is also responsible to sign the contract in person at the first appointment, but the online or verbal will serve as initial enrollment if the patient is not present for in-person appt when initial medical advice is offered or treamtent plan is ensued/developed.
